Research on Optimization Strategies for AI-Driven Community-Based Prevention and Control of Chronic Diseases

Yi Yuan, Di Wang, Fan Wang, Meng Yuan

Abstract

The threat posed by chronic diseases is increasingly becoming a great disease burden that threatens residents' health; however, as the
first line of defense for prevention and control, there are difficulties in managing it efficiently. With the emergence of artificial intelligence (AI)
technologies, the reshaping of community-based chronic disease prevention and control model becomes technically feasible. Multiple national
departments jointly issued a special implementation opinion in 2025 on establishing an intelligent chronic disease management and personal
health profiling application, providing intelligent services such as screening, risk assessment and grading, and personalized intervention. This
paper systematically summarizes the current practice of AI-driven community-based chronic disease prevention and control, analyses the
structural barriers existing in the process of technology enablement, and then proposes multiple optimization strategies covering data governance, service process reengineering, capacity building and institutional safeguards, which can be used as theoretical reference and practical
path for the intelligent transformation of community-based chronic disease prevention and control.
