further strengthen peoples scientific understanding of kidney disease and improve citizens health literacy. In the continuous service and management of CKD patients, based on the concept of 3 groups and 5 dimensions of CKD, we have established a set of renal health popularization and education system which is suitable for Chinese renal patients, easy to participate, and can be popularized, and we hope that through
the scientific and standardized health education, we can help the patients of CKD to understand renal disease scientifically, improve their selfmanagement ability, and assist the health care professionals and nurses to understand their kidney disease and to improve their self-management ability. Through scientific and standardized health education, we hope to help CKD patients understand kidney disease scientifically,
improve their self-management ability, and assist healthcare providers to carry out more efficient chronic management for target patients.
