Research Progress on the Application of Discharge Planning Services in Elderly Patients with Chronic Diseases
Abstract
rates of adverse events and readmissions. Discharge Planning Services (DPS) are a key strategy to mitigate these risks. This review synthesizes evidence on the core components of effective DPS and analyzes barriers to its implementation. Results: Effective DPS is a comprehensive
intervention including patient needs assessment, education, medication management, and service coordination. Implementation is hindered
by policy and payment issues, fragmented information systems, and personnel shortages. Conclusion: Improving outcomes requires shifting
from fragmented interventions to an integrated care ecosystem. This demands a concerted effort to optimize policies, integrate resources, and
ensure continuous patient support through better information exchange.
