The Effect of Continuous Nursing Combined with Diversified Health Education on Self-Management Efficacy in Hemodialysis Patients with Hyperphosphatemia

Abstract


Objective: To investigate the effect of a health management model integrating continuity of care and diversified health education
on the self-management efficacy of maintenance hemodialysis patients with hyperphosphatemia. Methods: A self-controlled study was per
formed on 193 patients who had received maintenance hemodialysis for no less than 3 months at Nanjing BenQ Hospital Affiliated to Nanjing
Medical University.All patients were initially provided with routine nursing and conventional health management. On the basis of the conven
tional regimen, an intervention combining continuity of care and diversified health education was implemented. During the inter?dialysis pe
riod, health education was delivered via video lectures, radio publicity and the distribution of promotional brochures; personalized education
strategies, including verbal instruction and customized health education prescriptions, were formulated according to each patients individual
conditions. During the peri?dialysis period, continuous nursing management was carried out through establishing a patient WeChat communi
cation group and organizing experience-sharing sessions among well?managed patients. Results: Dietary phosphorus intake in hemodialysis
patients with hyperphosphatemia was regulated through extensive publicity, targeted guidance and the implementation of continuous nurs
ing activities during the peri?dialysis period.Among the 193 enrolled patients, the serum phosphorus control compliance rate increased from
35.8% to 54.2%, 52.8% and 53.1% at different follow?up time points after intervention.A chi?square test yielded a ? value of 11.265 and a P
value of 0.001, indicating a statistically significant difference in the overall comparison between pre?education baseline and 5 months post?ed
ucation. Conclusion: Extensive health education, targeted guidance and continuous nursing interventions can significantly reduce serum phos
phorus levels and remarkably improve self-management efficacy in maintenance hemodialysis patients with hyperphosphatemia. Continuous
health education plays a positive role in enhancing patientsself-management efficacy.
