Innovative Models and Practices of Chronic Disease Management in the Context of an Aging Society

Abstract


With the intensification of population aging, chronic diseases have become the primary health issue for older adults, significantly
affecting their quality of life and imposing a heavy socioeconomic burden. Traditional chronic disease management models are struggling to
meet the increasingly complex needs of the elderly, necessitating the exploration and practice of innovative management approaches. This paper aims to discuss innovative models and practices of chronic disease management in the context of an aging society, including aspects such
as smart healthcare, community-based management, and multidisciplinary collaboration, in order to provide references for related research
and practice.
