The Application Progress of Transitional Nursing for Patients with Limb Dysfunction after Stroke

Abstract


Post-stroke limb dysfunction seriously affects patients quality of life, and transitional care (TCM), as a model designed to ensure
continuity of care during the transition of patients to different healthcare environments, has gradually gained attention in recent years in clinical application. This article reviews the theoretical basis of TCM, the current status of its application in patients with post-stroke limb dysfunction, its advantages, implementation strategies and challenges, aiming to provide reference for optimizing the rehabilitation nursing program
for stroke patients and improving the quality of nursing services.
