The Classification of Diabetic Macular Edema According to OCT Imaging Characteristics and Treatment Response Prediction

Abstract

Objective: To investigate the value of optical coherence tomography (OCT) imaging in the classification of diabetic macular edema
(DME) and its ability to predict treatment response to anti-vascular endothelial growth factor (anti-VEGF) therapy. Methods: This was a retrospective study of 92 eyes diagnosed with DME. OCT images were analyzed and classified into three morphological subtypes: diffuse retinal
thickening (DRT), cystoid macular edema (CME), and serous retinal detachment (SRD). Best-corrected visual acuity (BCVA) measured in LogMAR and central retinal thickness (CRT) before and after treatment were compared among different subtypes. OCT biomarkers including ellipsoid zone (EZ) integrity, external limiting membrane (ELM) status, and hyperreflective foci (HRF) count were assessed as predictors of treatment
outcome. Results: After three monthly intravitreal conbercept injections, CRT decreased significantly in all three subtypes (all P < 0.01). The
DRT subtype demonstrated the greatest improvement in visual acuity, while the CME subtype showed the least visual recovery. EZ integrity was
identified as the strongest predictor of favorable visual outcome (OR = 5.27, 95% CI: 2.14-12.96, P < 0.001). ELM integrity, HRF count, and baseline CRT were also significantly associated with treatment response. Conclusion: OCT-based morphological classification can effectively predict
treatment response to anti-VEGF therapy in DME patients, providing an imaging-based framework for individualized treatment decision-making.
